What You’ll Do Evaluate residents and develop individualized treatment plans in collaboration with residents, families, and the interdisciplinary care team Provide physical therapy services focused on restoring mobility, strength, function, and independence

Conduct screenings and identify residents who would benefit from intervention

Supervise and collaborate with PTA staff as applicable

Initiate referrals when appropriate in accordance with department procedures

Document clearly and efficiently in support of quality outcomes

What We’re Looking For Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program

Current PT license or license-eligible

Commitment to patient-first, outcomes-driven care

Team-oriented mindset with strong communication skills
