QUALIFICATIONS
- Education:
Minimum education is working towards an Associate’s Degree in Human Services or Bachelor’s Degree in Social Work

Competencies:
- Clinical skills including in systemic family therapy, crisis intervention, family education, and linking/ advocacy skills.
- Completion of Vroon Vandenburg High Fidelity Wraparound Training.

JOB SUMMARYThe Care Coordinator Intern is available to students actively enrolled in Human Services or similar Associate’s or Bachelor’s programs. The Care Coordinator Intern, under the administrative/clinical supervision of the CME Clinical Supervisor, is responsible for learning about care coordination and providing care coordination services to clients and families identified by the CME Catchment Area (CCA). The Care Coordinator Intern helps coordinate formal and informal services for the child and family in accordance with the principals of the High-Fidelity Wraparound model associated with the CME project.
This includes, but is not limited to, engaging families in Wraparound model, planning, implementing and monitoring service provision, and collaboration with involved partners.
