QUALIFICATIONS

  • Education:

    Minimum education is working towards an Associate’s Degree in Human Services or Bachelor’s Degree in Social Work
  • Licensure:
    None required.

    • Clinical skills including in systemic family therapy, crisis intervention, family education, and linking/ advocacy skills.
    • Completion of Vroon Vandenburg High Fidelity Wraparound Training.

    JOB SUMMARY

    The Care Coordinator Intern is available to students actively enrolled in Human Services or similar Associate’s or Bachelor’s programs. The Care Coordinator Intern, under the administrative/clinical supervision of the CME Clinical Supervisor, is responsible for learning about care coordination and providing care coordination services to clients and families identified by the CME Catchment Area (CCA). The Care Coordinator Intern helps coordinate formal and informal services for the child and family in accordance with the principals of the High-Fidelity Wraparound model associated with the CME project.

    This includes, but is not limited to, engaging families in Wraparound model, planning, implementing and monitoring service provision, and collaboration with involved partners.

    ESSENTIAL DUTIES
  • Observe and learn intensive care coordination and case management services provided to children and young adults (up to age 21) with severe and persistent mental health needs.
  • Shadow Care Coordinators in developing and maintaining service relationships with clients, families, and caregivers to understand engagement and support practices.
  • Participate in structured service planning and care coordination activities through High-Fidelity Wraparound, including observation and supported involvement in care plan development and monitoring.
  • Observe and assist Care Coordinators, under supervision, with coordination of referrals and linkage to appropriate community resources.
  • Learn documentation standards and confidentiality requirements related to client and family interactions, including legal, financial, and clinical records, in accordance with agency guidelines.
