The Home Health Registered Nurse (RN) is responsible for planning, coordinating, and documenting skilled professional nursing care provided 24 hours per day, 7 days per week for a geographically assigned caseload from admission through discharge. Utilizing physician's orders and professional skills, the RN functions as a case manager and implements a Home Health plan of care that meets each patient's needs while complying with State and Federal regulations, reimbursement guidelines, and Wellspring Home Health policies and procedures.
The RN also supervises home health aides, participates in weekend, holiday, and on-call rotations, and maintains a patient load of 5 to 7 visits per day.
- Nursing Process – Contribute to each patient's total plan of care through assessment, planning, implementation, and evaluation.
- Assessing and Planning – Conduct initial nursing assessments using the OASIS tool, develop individualized plans of care, collaborate with physicians, refer to other disciplines as needed, and communicate written and verbal plans with the home health team.
- Implementing and Evaluating – Facilitate nursing interventions, supervise aides, collaborate with therapists, social workers, and physicians, provide education to patients and families, resolve quality issues, and maintain accurate clinical documentation.
- Education & Mentoring – Provide educational experiences to staff, facilitate in‑services, develop teaching aids, and supervise aides and LPNs on‑site per policy.
- Manpower & Scheduling – Coordinate patient care assignments, schedule visits per acuity, conduct on‑call services, and manage aide and LPN supervision visits.
- Operational & Inventory – Ensure adequate supplies and equipment, promote efficient use of resources.
- Leadership & Communication – Demonstrate leadership behavior, support collaboration, maintain confidentiality, and communicate effectively with the entire home health team.
- Quality Improvement – Participate in quality assurance and performance improvement processes, pursue continuous education, and comply with all regulatory and policy requirements.
- Other Responsibilities – Conduct all business activities professionally, meet service standards, and complete a minimum of 12 hours of in‑services per year.
- Graduate of an accredited nursing program;
Baccalaureate in nursing or related field preferred. - Minimum of two (2) years of clinical nursing experience, with home health experience preferred.
- Excellent written and verbal communication skills.
- Current license as a Registered Nurse in the State of Ohio.
- Certified in Cardiopulmonary Resuscitation (CPR).
- Licensed driver with a working automobile and compliant insurance.
Must be able to ambulate throughout patients' homes, navigate stairs, operate a motor vehicle, travel in varying weather and road conditions, and handle stress effectively.
