The Case Manager (CM) will report to the Assistant Director as well as the site Program Manager for shelter coverage. The CM will be expected to work within the policies and procedures of Axess Family Services, Inc., and work toward the maintenance of good service standards and professional practices by utilizing the trauma‑informed care model. Each CM will be responsible for a case load of up to 25 veteran residents into permanent housing.
DUTIES AND RESPONSIBILITIES:- Complete assessments of veteran resident and/or veteran household’s needs and develop an appropriate support plan with the veteran, and if needed the family, focusing on obtaining or maintaining permanent housing.
- While providing services with a trauma‑informed care approach, the CM will assist with provision of a wide range of practical, personal, social, and general care services for the veteran resident and/or veteran household.
- Assist veteran residents and/or veteran households in developing realistic and achievable housing plans that focus on maintaining housing stability and include measurable action steps and timetables.
- Oversee monitoring, reviewing, and recording of the agreed‑upon support plan according to the changing needs of the veteran resident and/or veteran household.
- Promote veteran resident and/or veteran household involvement by maximizing their participation in the decision‑making process within the services.
- Communicate and liaise with various agencies (e.g., Department of Veterans Affairs, Veterans Services Commission, Probation Services, Housing Authority, Landlords, Department of Jobs and Family Services, Legal and Consumer Credit Organizations).
- Accompany veteran resident and/or veteran household to appointments with any of the above agencies as necessary.
- Maintain regular contact with the veteran resident and/or household including childcare assessments, crisis intervention, referrals, and ongoing life skills development.
- Ensure that veteran resident’s future and/or veteran household housing is habitable according to established organizational standards.
- Collect, provide, and update required data for the HMIS database.
- Support Social Service Workers and their duties in their absence or as needed.
- Provide staff support when on‑call.
- Assist in providing coverage for open shifts and call offs when on‑call.
- Travel to community locations where homeless and at‑risk veterans congregate to ensure effective local outreach.
- Promote anti‑discriminatory practice ensuring that no resident is discriminated against on the grounds of race, class, gender, religious beliefs, disability, or sexual orientation.
- Adapt to the environment you are working in; this could be part of a team or own initiative in a variety of settings.
- Apply Housing First, Trauma Informed Care, Harm Reduction principles and use Motivational Interviewing and De‑escalation Techniques in working with veterans of the program as appropriate.
- Attend meetings, trainings, and conferences as requested.
- Participate in community‑wide events promoting services for veterans.
- Foster positive working relationships with other staff members.
- Help SSSW, all staff, and veteran residents maintain up‑to‑date knowledge of internal fire regulations and procedures and other emergency procedures to ensure health and safety standards are maintained.
- Comply with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
- Maintain all records and logs.

MINIMUM QUALIFICATIONS:- High school diploma or equivalent required. Bachelor’s degree or pursuing a degree in social work and/or related field preferred.
