? Join our client's growing PACE program, an innovative care model delivering comprehensive medical services to high-risk seniors across Los Angeles. You'll be part of a collaborative team transforming how older adults experience healthcare.

• 2+ years experience as a Physician
• 1+ year in Geriatric Care, Senior Care, or Long-Term Care (LTC)
• Board Certified in Family Medicine or Internal Medicine
• DEA & CDS Registration in CA
• AHA Basic Life Support (BLS) Certification
• Valid California Drivers License, reliable personal transportation, clean driving record, and auto insurance
• PACE experience (1+ year)

• Deliver holistic primary care to PACE participants, including exams and chronic disease management
• Manage your own patient panel in coordination with a complex care RN
• Initiate and review orders for medications, diagnostics, and referrals
• Provide care across PACE center, participant homes, SNFs, ALFs, and hospitals
• Collaborate with Interdisciplinary Team (IDT) including behavioral health, pharmacy, and rehab
• Participate in care planning, Service Determination Reviews, and clinical team meetings
• Document accurately in the EMR
• Participate in after-hours phone call rotations, supported by nurse triage

Monday-Friday, 8 AM 5 PM

Flexible daily schedule with occasional travel to care settings

Rotating on-call (remote) coverage
