Clinical Pharmacist
Serve as an active member of the pharmacy team. Assist with development of clinical programs, clinical monitoring, medication reconciliation, formulary management, cost containment programs, and improved pharmacoeconomic outcomes. Build and enhance professional relationships with all departments, staff, and patients. Communicate effectively to promote clinical initiatives, provide patient care and education, and promote pharmacy initiatives. The Clinical Pharmacist will provide pharmaceutical care for patients of all ages: from infancy through geriatric.
Maintain a professional presence throughout the hospital. Assure pharmacy services and activities are carried out to meet goals and standards of the client/hospital.
Responsible for assistance in the development of the pharmacy driven inpatient diabetes stewardship and management program. Pharmacist III - Diabetes Care & Education Specialist will provide support to the department director, department managers, and staff by providing data derived from evidence based clinical best practice guidelines and latest published literature on the diabetes care. Perform the essential functions of a clinical pharmacist on a as needed basis.
PRIMARYPOSITION ROLES
- Collaborate with Pharmacy management, nursing, medical and ancillary health care team members to develop and maintain protocols and medication initiatives to:
- Reduce patient harm related to insulin administration
- Optimize institutional glucometric outcomes
- Ensure healthcare providers within the institution are up-to-date in their knowledge and approach to treating glycemic excursions in the hospital
- Enhance education efforts across all interprofessional service-lines within the organization
- Improve delivery of evidence-based pharmacotherapeutic recommendations
- Assist in evaluation of new diabetes medications for consideration for pharmacy formulary. Maintenance and implementation of pharmacy policies, protocols, and initiatives.
- Assist with pharmacy deliverables to hospital P&T, Medication Safety, Opioid Stewardship, Infection Control/Antimicrobial Stewardship, Heart Failure, COPD, and Diabetes Care Committee.
- Assist with the development and coordination of pharmacy medication use evaluations and report results to appropriate subsequent committee.
- Other functions as defined/delegated by manager.
- Perform as Staff Pharmacist as needed
- Maintains safety-integrity-professionalism of pharmacy at all times
- Perform special projects as requested
- Wears hospital badge identification while on duty.
- Meets dress code standards; appearance is neat and clean. Is conscientious in regard to personal hygiene.
- Treats patients and families with respect and dignity. Assures confidentiality of patient records.
- Maintains a good working relationship both within the department and with other departments.
- Assists in the resolution of problems or concerns while being a supportive team member.
- Consistently strives to promote great customer service in all aspects of the job.
A. Education
- Graduate of college of Pharmacy accredited by ACPE or equivalent - Pharm
D preferred
- Previous pharmacist experience or previous hospital pharmacy experience
- Board of Pharmacy Specialties Certification preferred.
- Three years related Hospital Clinical Pharmacy experience and/or Postgraduate Year One Pharmacy Residency preferred.
- Maintains active membership in American Society of Health-System Pharmacists or other Pharmacy-related State or National organization.
- Certified Diabetes Care and Education Specialist (CDCES) or equivalent.
- Training and proficiency in age specific therapies for hospital patients Neonatal, pediatric, adolescent, adult and/or geriatric
