At Healthcare in Action, we serve patients experiencing homelessness through an innovative street medicine approach, contracting with Medicare and Medicaid health plans, hospitals, and health systems. Street medicine involves using a medically equipped van to deliver medical care directly to those experiencing homelessness in parking lots, encampments, and parks. This is a limited‑term position of one year, dependent on performance and funding.
The Lead Peer Navigator (Liaison) will act as a liaison between HIA and the City of Los Angeles—Council District 4—collaborating closely to coordinate and enhance service delivery for individuals experiencing homelessness across the district. The liaison works with City staff in offices or in the field, as duties require, and the position is subject to change with funding.
We are seeking thoughtful, hard‑working, committed and compassionate team members to work in a collaborative, fast‑paced environment. Public engagement, field outreach, and government experience are valued, along with public speaking and consensus‑building experience. The Lead Peer Navigator (Liaison) may need to present at public meetings or build collaborative work groups with key stakeholders. Most important is that the candidate is self‑starting, organized, energetic, and dedicated to bringing about positive change in the lives of unhoused and housed constituents.
Responsibilities- Collaborate with Council District 4 leadership, staff, and service providers to align and streamline homelessness strategies and encampment interventions.
- Conduct daily outreach, encampment assessments, and wellness checks to support individuals experiencing homelessness across the district.
- Respond to inquiries, attend community meetings, and deliver updates on homelessness‑related efforts and policies.
- Present at meetings, lead work groups, and engage key stakeholders.
- Maintain accurate service records, analyze trends, and prepare regular reports to inform strategy and public communication.
- Coordinate sanitation responses and oversee events like Neighborhood Support Days to ensure services are delivered with dignity and impact.
- Other duties as assigned.
- G.E.D./High School, required.
- Bachelor’s degree preferred. 1–2 years of relevant work experience (direct, lived experience, training, or volunteer work with patients experiencing homelessness, active mental health or substance use conditions).
- Experience working with disadvantaged groups or outreach in a community setting is preferred.
- Comparable combination of education/experience and/or training considered equivalent.
- Compassion, commitment to and enthusiasm for addressing the concerns and needs of Council District 4 residents, both unhoused and housed.
- Ability to work in a dynamic, outdoor environment.
- Ability to work independently as well as part of a team.
- Ability to organize effectively and prioritize multiple competing tasks.
- Ability to communicate effectively, including articulating one's own relevant personal experiences.
- Excellent oral communication and strong interpersonal skills.
- Reliable transportation; travel 50% or more; must have a valid driver’s license, automobile insurance, clean driving record.
- Proficiency in operating a computer, including online timesheets and call‑center software.
- Knowledge of HMIS, Coordinated Entry System (CES) and Service Planning Areas 2 and 4 resources, preferred.
- Basic Spanish, preferred.
- Pay range: $32.69–$36.05 per hour
