The LA County Board of Supervisors recently declared a local state of emergency for homelessness. The County is working to revise and expand our response to help everyone affected. We are looking for qualified, passionate individuals to address housing, mental health, and substance use issues. This role directly benefits the population of LA County.

• Organize and coordinate activities of community groups to identify community resources and develop solutions for substance abuse, juvenile delinquency, homelessness, and mental illness.
• Receive service referrals from social workers, eligibility workers, medical case workers, other staff, or community stakeholders and help clients obtain food stamps, housing, medical services, mental health services, substance use disorder treatment, or other supportive services.
• Manage a caseload to help clients obtain employment training, jobs, housing, and other supportive services.
• Mentor Community Health Workers engaged in providing supportive services.
• Facilitate relations between County departments and the community by communicating strategic goals, priorities, policies, and procedures.
• Advocate for client access to County and community resources, ensuring preferences are included in treatment or recovery plans and reviewing plans timely.
• Document client services per departmental policies and procedures.
• Facilitate development of independent living skills by conducting support groups (e.g., housekeeping, cooking, shopping, budgeting, transportation, reading, personal hygiene).
• Perform outreach, engage clients, build rapport, identify needs, and provide resources or linkages.
• Assist clients, patients, and family members in completing forms for benefits, housing, jobs, and other services.
• Develop partnerships with diverse community and peer support groups.
• Liaise with local health care programs, schools, and social service agencies to create coordinated care and resource linkages.
• Organize, prepare, and maintain materials needed for community presentations, outreach, and events.
• Provide guidance to clients to assist in rehabilitation and recovery processes.

Minimum Requirements:

• Two years of experience at the level of Community Health Worker in a public or private community-based organization, public agency, hospital, public health organization or health system (cumulative experience allowed).

Community Health Worker Definition: Assists professional staff in providing direct health, mental health, or social services to clients and patients.

A valid California Class C Driver License or alternate transportation method.

Physical Class II – Light:
Light administrative and clerical work, occasional light lifting (up to 10 lbs), bending, stooping, or squatting, with considerable ambulation.

Under the emergency order, applicants who meet the requirements may be hired for an initial period of up to 90 days, with an opportunity for permanent County employment. Before the end of the initial work period, you will be assessed on your work performance (100%). Those who pass will be considered for permanent appointment.
