LVN Street Medicine

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide direct patient care in community-based and non-traditional settings. The LVN will work closely with a multidisciplinary team to deliver high-quality medical services, support care coordination, and promote wellness among underserved and vulnerable populations.

• Provide direct patient care under the supervision of a licensed physician or registered nurse.
• Administer medications, treatments, and wound care in accordance with clinical guidelines.
• Conduct health assessments, monitor vital signs, and accurately document patient information in electronic health records (EHR).
• Collaborate with care teams to develop and implement individualized treatment plans.
• Educate patients on health management, medication adherence, and preventative care.
• Respond to crises and assist with de-escalation as needed in challenging environments.
• Adhere to infection control protocols and ensure safe care practices in various field settings.
• Build rapport and establish trust with patients in a non-judgmental and supportive manner.
• Participate in outreach efforts to engage individuals in need of care, particularly those experiencing homelessness or other barriers to access.
• Coordinate referrals and resources with community-based organizations and social service providers.
• Maintain inventory and ensure readiness of medical supplies and mobile equipment.
• Follow all policies, procedures, and ethical standards in patient care delivery.

• Active and unrestricted LVN license in the state of California.
• Minimum of 1–2 years of clinical experience, ideally in community health, urgent care, or with underserved populations.
• Current BLS/CPR certification.
• Comfortable working in outdoor, mobile, or non-traditional healthcare environments.
• Valid driver’s license and access to reliable transportation.

Schedule/ Shift: Monday - Friday: 7:30/8:00am - 5:00pm
