The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.
The responsibilities of the Clinic RN include, but are not limited to:
Responsibilities:
- Conducts initial and quarterly assessment of each assigned participant's health and social status; identifies overt problems/needs and sets priorities; treats problems requiring immediate referral and/or follow-up; and interprets and records latest diagnostic results.
- In coordination with the Interdisciplinary Team, develops a Plan of Care based on nursing process that incorporates immediate and continuing care needs and includes the participant and family perspective.
- Provides care that conforms to accepted practice standards and age-related characteristics and needs of participants.
- Provides direct care that includes, but is not limited to wound care, IV therapy, blood sugar checks, IM injections, vital sign checks and/or follow-up on an episodic problem previously identified by the Primary Care Physician, such as Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infection or other health problems.
- Oversees the organization of the BCSC exam and treatment rooms and coordinates with other Interdisciplinary Team members to schedule participants in such a way as to assure day-to-day operational efficiency and participant well-being.
- When assigned, participates in daily morning meetings to update other Interdisciplinary Team members on changes in participants' condition and movement between home, supportive housing, nursing home, hospital and other residential settings. Provides briefing to Interdisciplinary Team members on participant health problems, daily schedules, planned tests, appointments, and Medical Specialty appointments scheduled.
- Records participant assessments, reassessments and care delivered in the medical record.
- Consults with and educates the participant, family members and/or caregiver regarding the disease process, self-care techniques, prevention strategies and medical interventions.
- Provides the Primary Care Physician with a summary of health findings and any changes in a participant's health condition.
- Ensures that all tests, lab work and diagnostic studies ordered by the Primary Care Physician have been carried out.
- Coordinates all medication for BCSC, including the stocking of non-prescription medications, the ordering of prescription medications, the administering of medication in the PACE Center, and the monitoring of participant compliance. Arranges for delivery of needed medications and supplies to the participant's home.
- Collaborates with the Primary Care Physician to assure that protocols are in place and standing orders are carried out with regard to bowel care; foot care; skin care; urinary tract infection; vital signs and weight; and wound care. Provides continence training to participants as needed.
- Prior to the use of physical or chemical restraints, conducts an assessment and documents in the medical record that participant restraint alternatives have been attempted and obtains a physician order for restraints. Obtains family consent or refusal for restraint use. Monitors and documents that the participant has a specific medical symptom that cannot be addressed by another less restrictive intervention and a restraint is required to treat the medical symptom, protect the participant's safety, and help the participant achieve his/her highest level of physical or psychological well-being.
- Supports the Infection Control function in coordination with the Quality Assurance Coordinator.
- Actively participants in the Quality Assessment and Performance Improvement (QAPI) process.
- Recognizes inappropriate participant care management or service delivery by other Interdisciplinary Team members and addresses directly and/or refers to the RN Clinic supervisor immediately.
