PositionWe are currently in need of an experienced Per‑Diem Hospice RN to give skilled nursing to hospice patients at home and in senior facilities. A minimum of one year of experience is required. The RN must have experience in hospice care and be able to bring each visit to completion with recording of the notes in the EMR system (Hospice MD).
CompensationOur current starting per‑visit rates are as follows:
- Supervisory Visits: $85 per visit
- Recertifications: $100 per visit
- New Admissions: $250 per admission
Our hospice patients are located in Long Beach and surrounding areas. The RN is expected to conduct supervisory visits, recertifications, and new admissions.
ResponsibilitiesThe RN will be responsible for the assessment, nursing diagnosis, and treatment of the assigned patients. The RN will initiate and coordinate the hospice care plan, supervise CHHAs and LVNs, and provide skilled nursing hospice and palliative care services. The RN will work cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team. The RN will regularly review and update the plan of care, perform prescribed medical treatments (including pain management and symptom control), conduct assessments and evaluations, and provide education and supportive care to patients and caregivers.
Must be able to utilize computers and be comfortable with EMR (Hospice MD). Hospice MD training can be provided.
- Current RN license within the State.
- Hospice experience required.
- Knowledge of EMR software.
