Primary responsibility is for the clinical case management of the transplant recipient involving all phases of the transplant process, from initial referral through patient follow‑up. This position requires an advanced level of expertise and overall understanding of organ transplantation for the specified organ type. Responsibilities include coordinating the interdisciplinary medical care delivery system under the direction of the Medical Director; serving as a liaison with patients, families, medical staff, referring physicians, and insurance companies;

providing individualized patient/family teaching; assessing appropriate medical interventions to use resources effectively and ensure quality assurance.

Additional responsibilities include data collection for clinical management and certification requirements; developing educational materials and programs for patients, nursing staff, and physicians; developing clinical protocols, policy, and procedures; engaging in community education and public relations; coordinating transplant donor/recipient processes and liaison functions with organ procurement agencies.

The Transplant Coordinator reports directly to the Nurse Manager. Clinical functions are supervised by the Transplant Program Medical Directors and the Nurse Manager. Overall job performance is evaluated jointly by the Transplant Program Medical Directors and the Nurse Manager.

• Current California Registered Nurse license.
• Graduation from an accredited nursing program with a B.S. degree or Master’s degree.
• Minimum 5 years of relevant RN experience with a combination of critical care and/or transplant experience (required).
• For TC II: CCTC or CPTC certification (required).
• Ability to case‑manage all aspects of a transplant patient’s medical care from initial evaluation to post‑transplant follow‑up (required).
• Broad understanding and knowledge of organ transplantation, including immunology, immunosuppression, rejection, and other complications, drug toxicity and graft failure (required).
• Effective communication skills with persons of various cultural backgrounds; good verbal and written communication skills (required).
• Effective interpersonal skills with patients, families, nursing staff, outside agencies, team physicians, and referring physicians (required).
• Excellent organizational skills, proficiency in meeting deadlines, ability to work independently, prioritize workload, manage multiple concurrent tasks in a high‑stress environment, and adapt to change (required).
• Ability to educate about transplantation and medical management issues (required).

Salary Range: TC1: $– annually; TC2: $– annually

The final candidate who accepts an offer of employment will be required to disclose any final administrative or judicial decisions within the last seven years determining that they committed any misconduct or any appeal of a finding of substantiated misconduct with a previous employer. Current/former UC employees are subject to a personnel file review.
