Full-Time Patient Care Coordinator (LVN)

Culver City, CA | Monday–Friday | In-Office Position

$35.00–$38.00/hr

We are seeking an experienced Patient Care Coordinator (LVN) to join our hospice team. This role is ideal for a compassionate, organized professional who thrives in a fast‑paced environment and is passionate about supporting patients and families through end‑of‑life care.

Key Responsibilities
  • Coordinate staffing, patient visits, admissions, and discharges
  • Support daily clinical operations and interdisciplinary team functions
  • Assist with clinical reporting, documentation, and compliance activities
  • Serve as a liaison between leadership, clinical staff, patients, and families
  • Maintain accurate records utilizing electronic medical records (EMR) systems
  • Provide administrative and clinical support as directed by leadership
Qualifications
  • Current California LVN license in good standing
  • Minimum one year of LVN experience required
  • Hospice/End‑of‑Life experience required
  • Strong computer and EMR proficiency
  • Excellent communication, organizational, and multitasking skills
  • Spanish speaking preferred
