Access TLC is currently seeking a HCBA Registered Nurse (RN) to join our team in the HCBA department. The Home and Community Based Alternatives (HCBA) Waiver provides care management services to persons at risk for nursing home or institutional placement. The care management team is composed of a nurse and social worker and coordinates WAIVER and State Plan services (medical, behavioral health, In‑Home Supportive Services, etc.)

and arranges for other locally available long‑term services.

• Assess, care plan, assist with locating, coordinating, and monitoring services for community‑based participants.
• Conduct initial assessment/reassessment visits and Level 3 follow‑up / Plan of Treatment visits on site in the Los Angeles area.
• Document findings and maintain communication with interdisciplinary team.

• Initial assessment/reassessment visits: $150 per visit.
• Level 3 follow‑up / Plan of Treatment visits: $50 per visit.
• Mileage reimbursed.

• Registered Nurse with current licensure to practice professional nursing in California.
• Minimum one year nursing experience.
• Excellent observation, verbal, and written communication skills.
• Current CPR certification.
• Valid California Driver’s License with proof of a good motor vehicle record.
• Current proof of auto insurance with state minimum or greater coverage.
