Position: Nurse Practitioner (Heart Failure) - Physician Extenders - Full Time 10 Hour Days (Exempt) (Union)

The Heart Failure Nurse Practitioner (NP) works under the direction of the Department's Administrative and Medical Directors to facilitate safe and efficient care of the Advanced Heart Failure/Cardiology patient population. The NP is responsible for applying the nursing process, supervising clinical practice, providing education and leadership, and managing patient care. The NP functions as part of inter‑professional teams by communicating, planning, and implementing care with physicians, pharmacists, office support staff, clinical nurse specialists, and other nurse practitioners.

Key Responsibilities
  • Assist and support the Cardiovascular Heart Failure Service Line to evaluate and treat Advanced Heart Failure patients including heart transplant, LVAD, and temporary mechanical circulatory support.
  • Round with the Advanced Heart Failure team in the ICU and the floor for primary and consult patients.
  • Respond to consult calls and document encounters (history & physical, new patient consult notes, follow‑up notes).
  • Admit patients from the office to the hospital; provide patient and family education, discharge education, and post‑discharge phone calls.
  • Arrange inpatient and outpatient procedures; prepare nightly sign‑out and coordinate outpatient follow‑up.
  • Comply with all medical center requirements, particularly goal‑directed heart failure therapy.
  • Continuously assess clinic needs, identify operational barriers, and ensure accurate documentation.
  • Communicate patient needs to providers promptly and maintain timely communication.
  • Delegate and direct activities, support staffing and operational needs, and update required licenses and credentials.
  • Modify current practices to improve operational flow and provide evidence‑based clinical decision making.
  • Model compassion, problem solving, conflict resolution, and public relations skills. Maintain up‑to‑date knowledge of clinical, departmental, and medical center changes.
  • Collaborate with other disciplines and providers to meet patient needs.
  • Follow policies and procedures, access online manuals, and apply the nursing process to patient care.
  • Perform patient assessments, formulate nursing diagnoses, develop plans of care, and document expected and projected outcomes.
  • Delegate tasks appropriately, act as patient advocate, and maintain accurate documentation in the Electronic Health Record.
  • Provide patient/family education, collaborate with case manager or social work for social needs, and utilize patient care equipment safely.
  • Manage time effectively, organize and prioritize responsibilities, and observe regulatory agency requirements.
  • Manage post‑cardiac procedure patients and participate in data collection and process improvement related to Nurse Sensitive Quality Indicators.
  • Perform additional duties as assigned, such as preceptor or peer education and unit activities.
Required Qualifications
  • Master's degree (MSN) from an accredited school of Nursing.
  • Minimum of two years experience as an RN or NP in Cardiology.
  • Minimum of two years experience in advanced heart failure, VADS, mechanical circulatory support, ECMO management, and heart transplant.
  • Strong critical, strategic, analytical, and planning skills.
  • Excellent organizational and time management skills.
  • Excellent communication skills.
Preferred Qualifications
  • Five years of NP experience.
Required Licenses/Certifications
  • Registered Nurse – RN (CA Board of Registered Nursing).
  • Nurse Practitioner – NP (CA Board of Registered Nursing).
  • Nurse Practitioner Furnisher (CA DCA) – current license to furnish medications in California.
  • DEA Certificate – drug‑enforcement agency certificate for ScheduleII–V controlled substances.
  • Board Certified in Nurse Practitioner specialty (ABSNC or NCCA), and for hospital/acute care settings, certification from ANCC or AACN as ACNP or AGACNP.
  • Basic Life Support (BLS) – Healthcare Provider from American Heart Association.
  • Advanced Cardiovascular Life Support (ACLS) – Healthcare Provider from American Heart Association (required for hospital/acute and infused clinic settings).
  • Fire Life Safety Training (LA City) – required if no card upon hire; must be obtained within 30 days and renewed before expiration.
