PositionSummary
USC Street Medicine is seeking a Licensed Vocational Nurse to work under the supervision of the Clinic Nurse Manager. The Licensed Vocational Nurse will join a multi-disciplinary team to provide care coordination and assist with transitional primary care for unsheltered homeless individuals living in Los Angeles.
Schedule & CompensationFull‑time, in‑person. M‑F, 8a‑4p. Hourly rate $31.11‑$35.00.
Key Responsibilities- Obtain accurate vitals and medical history.
- Conduct follow‑ups via phone calls, encampment visits, and visit other settings where patients can be found.
- Recognize changes in physical or mental condition and notify physician or nursing supervisor.
- Maintain files and records, oversee equipment and office supply inventory; perform clerical tasks such as photocopying, faxing, mail distribution, and errands.
- Document comprehensive medical records for billing, including patient history, medication list, allergy list, treatments and procedures.
- Promote security by alerting appropriate personnel for safety, fire and other unsafe situations; report any unusual occurrences to supervisor.
- Understand and apply crisis intervention, interviewing techniques, psychosocial problems, counseling and case management documentation.
- Provide individualized interventions to promote restoration of health and achievement of patient health goals.
- Understand age‑related and cultural needs of individuals served.
- Coordinate client care in a non‑discriminatory and non‑judgmental manner.
- Collaborate with clinical personnel and maintain excellent professional relationships with patients, visitors and staff; effectively communicate Street Medicine’s mission.
- Advocate on behalf of patients and liaise with community agencies to ensure continuity of care.
- Work closely with Street Medicine Registered Nurses, Nursing Supervisor, Clinical Coordinator, Physician/Physician Assistant/Nurse Practitioner, and other multidisciplinary team members.
- Perform additional duties as needed.
- High school diploma or equivalent.
- Completion of an accredited vocational nursing program.
- Passed the NCLEX‑PN licensing examination.
- One year of work experience.
- Valid California Vocational Nursing license.
- Valid BLS for Healthcare Provider.
- Valid IV/BW certification (IV therapy and blood withdrawal).
- Two years of experience working with the unhoused population.
- Demonstrated intrapersonal skills and knowledge of patient care and/or needs.
- Keen observational, decision‑making, and communication skills.
- Ability to follow orders and work independently within scope of practice.
- Related undergraduate study.
