Licensed Practical Nurse or Medical Assistant

Nurse (LVN or MA) — Adolescent Residential Treatment | Ascend Healthcare | Encino, CA

Ascend Healthcare is looking for a compassionate, detail-oriented Nurse to support two of our adolescent residential treatment programs in Encino, CA. Working closely with our Director of Nursing, you ll help manage the day-to-day medical needs of our clients, ensuring safe, timely, and high-quality care across both programs.

This is a hands-on role for someone who thrives in a fast-paced, relationship-driven environment and wants their clinical skills to make a real difference in young people s lives.

This position is scheduled on 8-hour shifts, either:

• Morning Shift: 7:00 AM – 3:30 PM
• Swing Shift: 3:00 PM – 11:00 PM

• Maintain accurate, timely client records — assessments, treatments, medication counts, vitals, and more
• Provide direct nursing care within your scope of practice and each client s individual treatment plan
• Monitor chronic health conditions and coordinate care with outside providers as needed
• Respond to urgent and emergent medical situations, including first aid, wound care, and stabilization
• Lead the intake process for new clients — paperwork, consents, and pharmacy coordination
• Maintain medication and supply logs, and manage inventory requests
• Uphold HIPAA, ADA, and Joint Commission standards across all documentation and care
• Support a safe, structured milieu across both programs, on and off site as needed

• Current LVN or RN license in California with 1+ year of nursing experience, or MA license/certification with 2+ years of MA experience
• Experience in residential nursing preferred
• EMR proficiency — familiarity with KIPU a plus
• Strong organizational, interpersonal, and decision-making skills
• A calm, professional, and compassionate presence — even in high-pressure moments
• Valid driver s license and ability to pass a federal background check and Livescan
• Reliable transportation for commuting between program sites as needed

LVN: $33–$38/hour, based on experience
MA: $23–$26/hour, based on experience

This role involves working directly with adolescents in residential treatment, some of whom may present with high-risk or unpredictable behaviors requiring active de-escalation and crisis response. The role also involves moderate to rigorous physical activity — standing/walking 4+ hours, lifting up to 50 lbs — and some exposure to varied environmental conditions. We provide training and support to help you navigate these moments with confidence.
