Monday through Friday, 8:00 am to 5:00 pm. No nights, weekends, or holidays.

Nurse Practitioner

• Evaluate participants and document information in participant’s chart.
• Perform physical examination and order and interpret diagnostic tests.
• Perform blood draws, EKGs, dressing changes and other nursing interventions as needed.
• Formulate an assessment of participant’s condition and provide a treatment plan, including writing prescriptions and providing ongoing follow‑up. Provide alternative methods of therapy as appropriate.
• Educate staff, participants and caregivers regarding healthcare issues and treatments.
• Manage acute and long‑term medical needs of participants in collaboration with other providers and oversee care in nursing homes, homes, assisted living facilities, and/or group clinic settings.
• Provide emotional and spiritual support to participants.
• Provide management of medical emergencies as needed.
• Ensure appropriate and timely documentation of medical care in the participant’s medical record, in compliance with Innov Age Greater PACE documentation policies. Identify and correct problems in medical records.
• Perform additional work functions and duties as assigned.

• Current State issued Nurse Practitioner license.
• 1+ year as a practicing Nurse Practitioner.
• DEA Certification with full prescriptive authority.
• 5+ years as a Registered Nurse with experience in elderly care.

• Bi‑lingual.

Compensation: $67.00 – $82.00 per hour.
