Licensed Vocational Nurse, Supervisor, Nursing Care Cooridinator

Job Overview

Responsible for the development and implementation of homecare services for program participants, including the coordination of all contracted home services, durable medical equipment, and nursing home care. Attending scheduled IDT and Coordination meetings.

• Graduation from an accredited LVN school, coupled with a current LVN license issued by the State of California.
• A minimum of one (1) year of experience practicing as a Licensed Vocational Nurse.
• Experience and knowledge regarding the physical, mental, and social needs of patients, especially with frail or elderly populations.
• Valid California driver’s license with a good driving record.
• Valid BLS certification or higher, following American Heart Association (AHA) or American Red Cross guidelines.

Pay for this job starts at $77,126.00 annually. Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations. The range is subject to change. This role may also qualify for discretionary bonuses or incentives.
