Health Care Synergy, Inc. is searching for a Clinical Case Manager to join its Managed Services – Clinical Team. As a Clinical Case Manager, you will oversee the implementation, evaluation, and supervision of the care provided to clients in their own homes.

• Review all OASIS Assessments.
• Generate Plan of Care.
• Ensure that Home Health Clinician’s Documentation meets Medicare Conditions of Participation and Rules and Regulations.
• Provide total oversight of Patient Care through documentation review – includes Home Health Clinician’s Observation and Assessment of patient’s disease condition, Management and Evaluation of the Plan of Care, Patient Teaching and compliance to Teachings.
• Ensure Care Coordination with the Physician regarding patient’s progress (or lack of), patient’s response to current doses of prescribed medications, and Medication Management and Reconciliation.

• 3 years relevant experience.
• College Graduate or Bachelor’s Degree.
• Fluency in English (Speaking – Reading – Writing).
• Able to follow policies and procedures and provide detailed documentation.
• Possess the ability to work independently.
• Exceptional understanding and working knowledge of Medicare Conditions of Participation, Rules, and Regulations required.
• Ability to make appropriate clinical judgments.
• Good written and oral communication skills; interpersonal communication and teaching skills.
• Ability to manage.
• Strongly preferred:
  • Home Health or Hospice experience
  • SOC RN, LVN/LPN, PT, OT, SLP licensure in any of the 50 US states
  • Certified Case Manager
  • Medical or Coding certification
  • Synergy EMR experience
• Salary Range: $45,000 – $52,000 per year
