• Provide direct patient care for ICU-level patients in a home hospice setting, delivering comprehensive assessment and monitoring.
• Administer prescribed medications and therapies, manage IV therapies, and respond to changes in patient condition with appropriate interventions.
• Monitor vital signs, track symptoms, and coordinate with the interdisciplinary team to adjust care plans as needed.
• Document care, interventions, and patient responses in Cerner and Epic electronic medical records; ensure accurate and timely charting.
• Educate patients and family caregivers on symptom management, safety, and end-of-life care planning; support families through the care trajectory.
• Adhere to clinical policies and safety protocols (including HLFS requirements) and maintain high standards of infection control and patient safety.

• Active registered nursing license in California
• BLS and ACLS certifications;
  Hospital Fire and Life Safety (HLFS) training
• Cerner and Epic experience within the last 5 years;
  Epic test-out option available
• Must have been an employee of an organization or have successfully completed a contract within the last 5 years
• Ability to work in a home healthcare - hospice setting and travel as required
