Position: Nurse Practitioner - Heart Acquisition - Full Time 8 Hour Days (Exempt) (Union)

The Nurse Practitioner (NP) delivers advanced clinical care for patients across the heart transplant and mechanical circulatory support (MCS/VAD) continuum, including pre‑transplant evaluation, waitlist management, post‑transplant follow‑up, and durable VAD therapy. The NP works collaboratively within multidisciplinary transplant and MCS teams in both inpatient and outpatient settings, ensuring safe and high‑quality patient care.

Responsibilities
  • Constitute comprehensive clinical assessments and participate in selection committee activities.
  • Support cross‑coverage across transplant and MCS services.
  • Contribute to staff and patient education, and maintain accurate documentation for regulatory and reporting requirements (CMS, TJC, INTERMACS, UNOS/UNet).
  • Participate in quality improvement initiatives and operational needs of the program.
  • Provide on‑call coverage as needed.
Essential Duties
  • Conduct in‑depth interviews with patients and/or families, construct medical histories, and perform physical examinations.
  • Identify and treat patient diseases, manage treatment using analytical and planning skills.
  • Identify educational needs and provide health‑related information to patients and families.
  • Instruct and counsel patients regarding care, medical problems, psychological issues, and use of devices and medications.
  • Perform or order diagnostic studies, interpret results, and develop treatment plans.
  • Obtain informed consent for procedures and initiate consultations as required.
  • Maintain progress records, update charts, and notify responsible providers of changes.
  • Act as liaison with governmental agencies such as CDC and NIH when indicated.
  • Assist with medical research projects, obtain IRB approval, and conduct research protocols.
  • Refer patients to community resources based on medical needs.
  • Ensure compliance with institutional policies, quality assurance, safety, infection control, and accreditation regulations.
  • Participate in in‑service education programs and professional associations.
  • Perform other duties as assigned.
Required Qualifications
  • Master’s degree from an accredited school of Nursing (MSN).
  • Minimum five years of clinical nursing experience.
  • Excellent communication skills with proficiency in English.
  • Knowledge of medical procedures and ability to apply them effectively.
Preferred Qualifications
  • Three years of experience as an NP in the specified specialty area.
Required Licenses and Certifications
  • Registered Nurse - RN (CA Board of Registered Nursing).
  • Nurse Practitioner - NP (CA Board of Registered Nursing).
  • Nurse Practitioner Furnisher (CA DCA) – current license to furnish medications in California.
  • DEA Certificate for Schedule II‑V controlled substances.
  • National Board Certification in the NP’s specialty area (ABSNC or NCCA). (Exempt if hired/transferred before 12/2011)).
  • Acute Care Nurse Practitioner (ACNP) or Adult Gerontology Acute Care Nurse Practitioner (AGACNP) certification from ANCC or AACN for hospital/acute care settings (grandfathered if hired/transferred before 12/2015).
  • Basic Life Support (BLS) certification from the American Heart Association.
  • Advanced Cardiovascular Life Support (ACLS) certification from the American Heart Association.
  • Fire Life Safety Training (LA City) – required within 30 days of hire if not already held.
