As an Inpatient Registered Nurse Care Manager, you will provide comprehensive, patient‑centric care management services for at‑risk patients in our acute care environment. Key accountabilities include assessment and planning, coordination of care, resource utilization management and review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.

• Conduct assessments and develop care plans for at‑risk patients.
• Coordinate multidisciplinary care and manage resources.
• Review and manage resource utilization.
• Plan and facilitate patient discharge.
• Document interventions and maintain regulatory compliance.
• Advocate for patient needs and preferences.

Required

• Associate’s Degree in Nursing (degree/diploma) upon hire.
• California Registered Nurse License upon hire.
• California Fire and Life Safety Card within 30 days of hire.
• 2+ years of Acute Care experience (Medical Surgical or Inpatient setting).
• 1+ year of care management or utilization review experience, or completion of TIPS program or Case Management Orientation Program.

Preferred

• Bachelor’s Degree in Nursing or higher within 3 years of hire.
• Case‑management certification (ACM or CCM).
