Overview
Registered Nurse to join Astoria Healthcare Center in Sylmar, CA. Provide high-quality, patient-centered care to residents and collaborate with an interdisciplinary team.
Responsibilities- Provide direct patient care, including administering medications, wound care, and treatments
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Conduct initial and ongoing assessments, documenting patient status and progress
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
- Participate in quality improvement initiatives and maintain accurate records and reports
- Current Registered Nurse licensure in the state of California
- Additional education and/or experience as required by the facility
