Licensed Practical Nurse

Duties and Responsibilities include but are not limited to:

Advanced Clinical Care:

• Provides a full range of nursing care for Veterans with complex medical, cognitive, behavioral, or chronic conditions.
• Recognizes subtle clinical changes and initiates timely responses.
• Performs advanced clinical tasks, including:
• Wound care (complex dressings under RN/provider delegation).
• Catheter care and identification of catheter-related complications.
• Enteral feeding administration and monitoring.
• Tracheostomy care assistance (facility-specific).
• Oxygen therapy and respiratory support within the LVN scope of practice.
• Conducts focused assessments, documents findings, and determines when results require prompt RN/provider review.

Care Planning & Prioritization:

• Participates in planning nursing care for a group of residents using established procedures and clinical judgment.
• Provides resident-specific data and identifies effective or ineffective interventions.
• Recommends modifications to care plans in response to clinical or behavioral changes.
• Prioritizes nursing care based on resident needs and changing conditions.

Medication Administration:

• Administers medications safely using the Bar Code Medication Administration (BCMA) system, including oral, topical, injectable, inhaled, and certain IV-related tasks within the LVN scope of practice.
• Identifies adverse reactions and takes appropriate action.
• Educates residents and caregivers on medication purpose, administration, and potential side effects.

Resident-Centered CLC Care:

• Provides trauma-informed, dignity-centered, age-friendly care consistent with the Community Living Center (CLC) philosophy of promoting independence, comfort, and engagement.
• Supports restorative nursing programs, mobility plans, and person-centered goals.
• Manages residents with dementia-related behaviors using non-pharmacological interventions and safety techniques.

Interdisciplinary

Collaboration:

• Works closely with Registered Nurses, Nursing Assistants, providers, therapists, social workers, dietitians, and CLC leadership.
• Communicates clinical observations that contribute to interdisciplinary decision-making.
• Participates in interdisciplinary team meetings, care conferences, and shift huddles.

Documentation & Communication:

• Documents nursing care accurately in CPRS and BCMA in accordance with VA and facility policies.
• Ensures documentation is timely, clinically accurate, and complete.
• Communicates urgent or significant changes in resident condition to the supervising RN or provider.

Safety & Compliance:

• Applies infection prevention and control practices, including appropriate use of PPE and isolation precautions.
• Uses safe patient handling equipment correctly and promotes safe handling practices.
• Identifies and reports environmental hazards, equipment malfunctions, and safety concerns.
• Participates in quality improvement and patient safety initiatives.

Leadership & Mentorship:

• Serves as a role model for Licensed Vocational Nurses and Nursing Assistants.
• Assists with orientation and skills training of new staff.
• Demonstrates leadership by promoting teamwork, professionalism, and high-quality patient care.

Pay:
Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
