• Deliver coordinated nursing care for a patient or assigned group of patients per established standards and the nursing process.
• Assess patients, analyze data, and determine care issues; develop and implement a care plan to achieve outcomes.
• Coordinate with the treatment team, family, and other disciplines to ensure safe, effective, and compassionate care.
• Monitor patient status, administer medications and treatments as appropriate, and document progress and outcomes.
• Supervise and direct activities of various nursing staff levels; delegate duties and ensure effective team collaboration.
• Participate in discharge planning and patient/family education to promote continuity of care and safety at home.

• Active Registered Nurse license in the state of California
• Bachelor’s Degree in Nursing (BSN) preferred
• Minimum of 2 years of recent experience in the field; acute care facility experience preferred
• Current Basic Life Support (BLS) certification from an approved vendor;
  Cardiopulmonary Resuscitation (CPR) certification noted as required
• ACLS and PALS certifications from approved vendors as required by policy
• Ability to work in a home health/hospice setting
• Spanish speaking is a plus

• Guaranteed hours: 36 per week
• Travel assignment with a 13-week contract
