About the role
Under the direct supervision of the Associate Director, Community Partnerships, the Lead Registered Nurse (LRN) will assist the Associate Director with managing a team of nurses, providing supervision and clinical guidance, ensuring adherence to healthcare protocols, and promoting patient well-being through effective leadership and clinical expertise. This role requires a combination of excellent direct patient care skills and team management skills.
Responsibilities include assisting with scheduling and supervising Health & Faith Partnership Sites, providing direct patient care, and leading a collaborative and effective culture for the Health & Faith Partnership team. The LRN will also ensure that Nurses are up to date on needed trainings and clinical competencies to support quality patient support initiatives.
What you'll do
ESSENTIAL JOB RESPONSIBILITIES Oversight of Community Nurses and Clinical Practice- Serves as the primary clinical resource for nursing staff by providing day-to-day guidance on patient care, clinical procedures and regulatory requirements.
- Supports daily nursing operations by assisting with workflow, resolving routine clinical issues, and promoting efficient patient care delivery.
- Assists with onboarding, orientation, and training of nursing staff and reinforces clinical competencies, including non-diagnostic screenings and laboratory equipment procedures.
- Monitors compliance with established clinical protocols, infection prevention practices, and applicable Department of Public Health and Los Angeles County requirements, escalating concerns as appropriate.
- Assists with vaccine ordering, inventory, storage, returns, and documentation in accordance with applicable regulations.
- Coordinates required clinical documentation in support of renewing Non-Diagnostic Health Asssessment (NGHA) permits and CLIA licenses.
- Coordinates with administrative staff to maintain clinical equipment, schedule calibrations, arrange sharps and medication disposal, and ensure clinic readiness.
- Participates in quality improvement activities by identifying opportunities for process improvements and communicating recommendations to management.
- Serves as a liaison between nursing staff, community partners and leadership to promote effective communication and continuity of care.
- Coordinates the daily work of nursing staff to ensure efficient service delivery and high-quality patient care.
- Prepares daily staff schedules, coordinates meal and rest break coverage, and adjusts assignments based on operational needs.
- Reviews and approves timecards, manages day-to-day performance in accordance with organizational policies, and escalates performance concerns and exceptions to management.
- Provides coaching and direction, reinforces established clinical and operational procedures, and communicates operational concerns to management.
- Coordinates nursing coverage, community events and operational readiness for new and existing service locations.
- Monitors and documents service site issues, supply needs, and recommendations, and escalates significant concerns to management.
- Performs other duties as assigned.
- As needed, conducts preventive health screenings, wound care, and administers vaccines within scope of practice.
- Provides health education, counseling, and preventive health interventions at community events and assigned service locations.
- Acts as a clinical advocate by identifying barriers to care and coordinating referrals to Community Health Workers and community resources.
- Maintains compliance with CDC infection prevention standards, Los Angeles County requirements, and organizational policies for clinical practice, vaccine management, and documentation.
- Maintains accurate and timely documentation in the Electronic Health Record (EHR) and completes required NGHA proficiency testing.
- Collaborates with internal programs and community partners to coordinate services and improve access to care.
