Acute Care Case Manager

ScheduleFlexibility in scheduling is required.
Assignment DetailsTravelers must be flexible with their schedules.
Requirements- Minimum 3 years acute care case management experience
- Valid California RN license active at the time of submission
- Basic Life Support (BLS) certification
- Experience with Epic preferred
- 2 references from within the past year (supervisors or higher)
- Manage acute care patient cases effectively.
- Coordinate care and discharge planning for patients.
- Conduct utilization review and ensure compliance with standards.
- Familiarity with CMS/Core measures and HIPAA regulations.
- Collaborate with interdisciplinary teams to enhance patient outcomes.
Mandatory modules must be completed, with up to 40 hours required, 17 hours of which are billable. Max return time off (RTO) is 2 weeks total; no RTO within the first 2 weeks of assignment.
