- Provide home health care to geriatric patients during a 9-week episode
- Assess patient needs
- Work with the RN case manager and Director of Nursing
- Deliver quality, personalized patient care in patients' homes or residential facilities
- Arrange visits directly with patients according to mutual availability
- Typically conduct visits lasting 45–60 minutes
- Travel within the assigned territory and provide coverage as needed
- Active LPN License in the state of practice
- Valid driver license
- Valid auto insurance
- CPR or BLS Certification
- Valid TB test (skin test within the last year or X-ray within the last five years)
- Willingness to travel to patients' homes or residential facilities
- Open availability to patients as needed
- New graduates or seasoned nurses welcome
- Home Health experience is not required
Demonstrates the ability to provide quality home health care to geriatric patients, including patient assessment and collaboration with healthcare professionals. Holds essential certifications and licenses to ensure compliance and effective patient care delivery.
