Under the direction of the Program Manager, the Drop-In Center Case Manager provides high-quality, center-based case management services to individuals and families accessing support through the Drop-In Center. This role serves as a primary point of contact for participants, promoting stabilization, service connection, and long-term self-sufficiency through structured, on-site services in a safe and welcoming environment.
This position is fully center-based. Services are provided on-site at the Drop-In Center only. No fieldwork, outreach, or home visits are required.
Key Responsibilities- Conduct client intakes and comprehensive needs assessments
- Complete psychosocial assessments and develop individualized service plans in collaboration with participants
- Provide trauma-informed, on-site case management services
- Assess client needs and connect participants to appropriate community resources
- Coordinate services including housing referrals, healthcare access, benefits enrollment, employment resources, and social services
- Maintain accurate and timely documentation, case notes, and client records
- Build and maintain trusting, professional relationships with participants
- Support clients in achieving stability goals related to housing, health, income, and self-sufficiency
- Coordinate care with internal staff and external service providers
- Utilize harm reduction, strengths-based, and client-centered approaches
- Manage an active caseload of Drop-In Center participants
- Commitment to the mission, vision, and values of The Good Seed CDC
- Knowledge of homelessness, poverty, and social service systems
- Strong interpersonal, organizational, and communication skills
- Ability to work effectively with diverse populations experiencing complex needs
- Experience conducting intakes, assessments, and service planning
- Familiarity with Los Angeles County social services and community resources
- Trauma-informed, culturally responsive, and client-centered approach
- Ability to thrive in a fast-paced, high-volume environment
- Flexible schedule, including occasional evenings and weekends as needed
- Experience with case management documentation systems preferred
