The Social Worker Program Coordinator is an independently licensed Advance Practice–level professional for Health Care for Homeless Veterans (HCHV) Contracted Residential Services (CRS). The HCHV CRS Liaison monitors and coordinates services provided by community‑based programs funded under VA emergency housing contracts.

• Provide supportive services and case management to Veterans residing in Contract Emergency Residential Housing facilities.
• Utilize various treatment modalities in individual and group work—including casework, community organization, and outreach—to alleviate or remove problems interfering with the Veteran's ability to achieve independence.
• Monitor clinical care and assess compliance of the programs receiving payments, as described in the recipient's contract.
• When necessary, intervene to facilitate compliance or corrective actions and, when appropriate, develop, facilitate, and monitor plans of correction.
• Provide oversight of the Contract Residential Services as defined by VHA policy.
• Serve as the Contracting Officer Representative (COR) for the Contract Emergency Residential Housing Program upon completion of required training and certification.
• Coordinate care and act as an advocate on behalf of Veterans to ensure comprehensive service delivery and provide linkages to VHA health‑care services, the Veterans Benefits Administration, and community agencies.
• Provide a wide range of services—including assessments, referrals, short‑term case management, advocacy, and service coordination.
• Provide a full range of social work services within commonly accepted standards of practice, including individual, family and group counseling, discharge planning, case management, and other appropriate services.
• Provide consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health‑care adherence and housing plans.
• Participate in team conferences to promote professional development, training, and coordination of service delivery.
• TRAVEL REQUIRED:
  
  Occasional car travel to Veterans Contract Residential Services Programs or in the community to assist Veterans with housing and conduct site visits of contract agencies.
