Youth Advocate Programs, Inc. is seeking a full time Case Manager. As a Case Manager, you will be responsible for providing comprehensive case management services to individuals and families in need. You will work closely with participants to assess their needs, develop individualized service plans, and connect them with the appropriate resources and support systems.
Status: Full-Time Hourly
FLSA Classification: Non-Exempt
Summary of Position: Full Time Case Manager will provide intensive individual mental health interventions and support services. The Case Manager is responsible for providing comprehensive case management services to individuals and families. Will work closely with participants to assess their needs, develop, and implement individualized service plans, and connect the with the appropriate resources and support systems.
Qualifications/Requirements:
- Master’s degree preferred;
Bachelor’s degree in social work, psychology, or a related field. - Experience working in a community-based agency program.
- Proven experience in case management or similar role
- Strong knowledge of social issues and community resources
- Excellent interpersonal and communication skills
- Organized and has good time management skills.
- Proficient in Microsoft Office 365
- Knowledge of Electronic Health Record Systems (EHR) is a plus.
- Bilingual (Spanish) is a plus
- Reliable transportation, valid driver's license, and current auto insurance coverage is required.
