Youth Build empowers young people ages 16–24 who've faced barriers to education and employment, combining classroom learning, hands‑on job training, and leadership development to help them earn credentials and build lasting careers.
The Care Manager guides Youth Build participants from enrollment through program completion, coordinating individualized care plans, providing comprehensive case management, connecting participants to health, housing, and social services, and providing day‑to‑day coaching that helps young people build the skills and stability needed to reach their goals.

DUTIES AND RESPONSIBILITIES:- Conduct outreach and marketing to recruit participants and educate the community about ACE services.
- Conduct intake, eligibility screening, and referrals for applicants who do not qualify.
- Complete program assessments to guide enrollment and individualized care planning.
- Develop and update individualized care plans, focusing on program performance and coordinating health, housing, and social services in compliance with funder and ACE standards.
- Coordinate with education and vocational instructors to align support services with participants’ program progress.
- Provide day-to-day coaching to reinforce program expectations and support participants’ progress toward goals and skill development.
- Provide crisis intervention support and connect participants to mental health, substance use, or other clinical services as needed.
- Organize, coordinate, prepare, and deliver program components such as skills‑based workshops and life skills classes; manage related program agreements and documentation.
- Support participants in building leadership skills and engaging in community service activities.
- Provide transportation for participants as needed to meet program goals.
- Maintain accurate, timely, and compliant participant records in program databases and support program reporting.
- Create and maintain secure, compliant participant files to ensure continuity of service delivery.
- Incorporate best practices in diversity, equity, inclusion, restorative justice, and trauma‑informed care into daily practice.
- Other duties as assigned.

- General proficiency in navigating and using technology.

- Sensitivity to the community, culture, and personal dignity of participants; able to engage, assess, and plan services effectively for the target population.
- Maintains appropriate professional boundaries with participants and their families.
- Knowledge of accurate participant record‑keeping practices, including timely charting, proper notation formats, and confidentiality requirements.
- Familiarity with programs, regulations, and procedures related to human services, social services, justice, and workforce development.
- Prolonged periods standing and walking throughout facilities.
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
- Must be able to bend, lift, stretch, climb, and crawl to provide services.
- Associate's Degree in a relevant field and 2 years of human service experience; or an Equivalent combination of training and experience.
- Spanish language skills, verbal and written preferred.
- Possess a current California Driver’s License, clean DMV record & adequate auto insurance on a reliable vehicle available to use for work purposes.
- Must submit to a Department of Justice Criminal background check.
- Initial TB employment clearance required.
