PATH is seeking a Case Manager – Housing Navigation in Los Angeles, CA to support clients in breaking the cycle of homelessness. This full-time, non-exempt role requires a compassionate, detail-focused professional who can coordinate service plans, provide referrals, and document progress in a timely manner.
