For over 30 years, the Violence Intervention Program (VIP) has been the leader in building transformative programs that raise the standard of care for all victims of child abuse, neglect, and sexual assault in Los Angeles County. Today, VIP functions as a public-private partnership between the publicly funded VIP Children’s Medical Village at the LAC+USC Medical Center and our non-profit, the VIP Community Mental Health Center, Inc.

Position Summary

The Case Manager is a core member of an FSP-ICM (Full-Service Partnership – Intensive Case Management) team providing intensive, community-based, whole-person behavioral health services to adults with serious mental illness (SMI) and/or co-occurring substance use disorders (SUD). This position is assigned to the Housing Supportive Services Program (HSSP), a specialty FSP-ICM model that provides field-based mental health case management to formerly unhoused individuals living in tenant-based or project-based Permanent Supportive Housing (PSH).

HSSP operates as part of an integrated services partnership among the Department of Mental Health (DMH), Department of Homeless Services and Housing (Intensive Case Management Services, ICMS), and substance use disorder providers, working together to support housing retention, mental health recovery, and reduced reliance on crisis, hospital, and justice-system services.

Essential Duties & Responsibilities Care Coordination & Case Management
  • Serve as a Primary Care Coordinator for an assigned caseload, acting as the main point of coordination among the multidisciplinary FSP-ICM/HSSP team for each client.
  • Participate with all other team members in outreach, engagement, and enrollment activities for individuals referred through the Service Request Tracking System (SRTS), including referrals from HD-HSI, ICMS, SUD providers, and property management.
  • Collaborate in needs assessments, treatment planning, resource coordination, crisis intervention, and skill development to help clients with complex needs access and navigate medical, social, employment, educational, and other support services.
  • Support clients not on the primary caseload as needed, consistent with the team-based service model.
  • Deliver a minimum of four face-to-face contacts per month per client (more frequently as clinically indicated), with at least 25% of all direct services delivered in field-based settings (75% for unhoused, recently housed, or reentry populations, unless the client declines field-based services).
  • Document all referrals, outreach contacts, and engagement efforts in SRTS and other required systems (e.g., HSSP Tracker, IBHIS/HMIS) in a complete, accurate, and timely manner.
Housing Support & Retention (HSSP – Specific)
  • Support delivery of housing services, including housing retention and other recovery-oriented services, for clients residing in PSH.
  • Assess mental health symptoms and behavioral manifestations that contribute to housing instability, and coordinate with the team and service partners on a care plan to address them.
  • Provide home visits at the frequency required to support housing retention and meet housing subsidy requirements.
  • Respond promptly to lease violations or landlord/property manager concerns to help clients avoid eviction; assist with legal services, subsidy renewals, and representative payee arrangements as needed.
  • Work with landlords, property managers, developers, and facility managers to advocate for and support client housing retention.
  • Assist clients with relocation when necessary.
  • Contribute toward program goals of 90% housing retention at 12 months and 80% at two years for clients served.
Clinical & Recovery Support
  • Assist with needs assessments related to housing, employment, daily living activities, and substance use.
  • Support delivery of integrated behavioral health care for clients with co-occurring SMI and SUD, in coordination with the team's SUD Specialist and clinical staff.
