Social Worker Per Diem

Location:

North Hollywood, CA. Shift: Monday‑Friday 8am–4:30pm. No evenings, no weekends.

Welbe Health provides primary care and social services to frail seniors. The Social Worker works within an interdisciplinary team (IDT) to support participants in achieving their full potential.

• Conduct psychosocial assessments to determine the needs, preferences and goals of participants and actively participate in IDT meetings to develop care plans.
• Deliver and document social‑work interventions as agreed upon in participants’ care plans, including arranging resources and services, assisting with care transitions, providing individual or group counseling and case management.
• Collaborate with the primary care physician and other team members to guide smooth transitions between settings.
• Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among team members, participants and their support network.
• Provide discharge planning when participants disenroll from the program.

• Master’s degree in Social Work (MSW) required.
• Minimum one year of experience with a frail or elderly population.
• Experience coordinating and facilitating care conference meetings.
• Experience assisting people with behavioral health and substance abuse issues (preferred).

Hourly rate $35.87‑$47.36, plus bonus and equity. Total rewards include 401(k) match, healthcare coverage and other benefits. Pay adjusted based on experience.

Proof of vaccination, or a vaccine declination form, is required before employment. All staff must meet required infection control and PPE standards.
