Job Summary

We are seeking a compassionate and detail-oriented Case Manager for a direct hire opportunity with a mission-driven nonprofit organization serving female veterans in Los Angeles, CA. This role supports veterans as they work toward greater stability, independence, housing security, employment readiness, and access to community resources.

The Case Manager will work under supportive supervision from program leadership, including a Program Coordinator, Clinical Director, or senior Case Manager. This is a strong opportunity for someone with 1 to 2 years of case management, social services, nonprofit, housing, workforce, or veteran services experience who wants to grow in a meaningful, community-focused environment.

Key Responsibilities
  • Assess client needs and barriers related to housing, employment, education, finances, sobriety support, legal concerns, family support, and medical or psychiatric services.
  • Coordinate services with VA personnel, community partners, housing providers, employment resources, and internal program teams.
  • Complete client intake, maintain confidential case files, document progress notes, update Individual Action Plans, and prepare discharge summaries.
  • Conduct home visits, client meetings, life skills support, and service referrals in accordance with program requirements.
  • Enter accurate and timely data into HMIS and other internal or external databases while maintaining HIPAA compliance.
  • Support program-specific services related to transitional housing, permanent housing, workforce development, rapid rehousing, homeless prevention, or incarcerated veteran transition services as assigned.
Compensation and Benefits
  • Direct hire opportunity.
  • Compensation: $23 to $25 per year.
  • Schedule may be 8:00 AM to 5:00 PM or 8:30 AM to 5:00 PM.

  • Onsite parking is first come, first served, with street parking available as needed.
Required Qualifications and Skills
  • Provide supervised case management services that support client independence, housing stability, and access to services.
  • Maintain accurate client files, progress notes, Individual Action Plans, discharge summaries, and program documentation.
  • Coordinate care with VA contacts, community agencies, housing resources, and internal team members.
  • Support client intake, assessments, referrals, home visits, life skills activities, and program compliance.
  • 1 to 2 years of relevant experience in case management, social services, nonprofit programs, administrative support, housing services, workforce services, or a related field.
  • Strong documentation, data entry, file management, and administrative skills.
  • Ability to maintain confidentiality and follow HIPAA standards.
  • Comfortable working with vulnerable populations and providing professional, trauma-informed support.
  • Ability to communicate effectively with clients, team members, community partners, and agency representatives.
  • Strong organizational skills with the ability to manage deadlines, documentation, and follow-up tasks.
  • Ability to attend team meetings, trainings, and complete required reports and documentation accurately and on time.
  • Ability to conduct home visits and support clients in community-based settings.
  • Staff with a valid driver’s license who meet insurance requirements may be asked to complete driver training and drive company vehicles for work-related activities, including client transportation.
Preferred Qualifications
  • Experience working with veterans is preferred.
  • Experience working with homeless individuals or families is preferred.
  • Experience with transitional housing, permanent housing, rapid rehousing, homeless prevention, workforce development, or supportive services programs is a plus.
  • Experience using HMIS or similar case management databases is helpful.
  • Experience supporting clients transitioning from jail or prison is preferred for incarcerated veteran transition program assignments.
  • Ability to pass background clearance for jail or prison access may be preferred or required for specific program assignments.
