As part of a larger Homeless Prevention team with multiple programs. Adult Protective Services (APS) Integrated Care Services (ICS)/Home Safe Social Worker I is responsible for providing care coordination, housing-focused case management, and supportive services to older adults age 60+ and dependent adults age 18-59 referred by APS who are experiencing homelessness, at risk of homelessness, or in need of short-term support to maintain safety, housing stability, and independence.

The Social Worker I will provide services that include but are not limited to enrollment; assessment; home visits; information and referrals; housing stabilization; landlord mediation; eviction prevention support; assistance with budgeting and/or money management; assistance with accessing government benefits; developing a Housing and Services Plan; and coordinating client relief and housing-related financial assistance to help clients maintain safety and long-term housing stability

Qualifications
  • BA degree in Social Work or a related social services or health-related field.
  • Prior experience providing case management, care coordination, housing stabilization, and community-based service delivery experience with the older adult population preferred.
  • Knowledge of community resources, housing resources, public benefits, eviction prevention resources, aging/disability services, and landlord/tenant rights preferred.
  • Excellent verbal and written communication skills and ability to utilize telehealth platforms.
  • Experience conducting field-based home visits and community-based service delivery strongly preferred.
  • Knowledge of HMIS or other record database management systems preferred.
  • Must be computer literate in MS Office applications, including Word, Excel, Outlook, SharePoint, Teams, and ability to learn new systems.
  • Ability to work with clients who may have complex needs, including cognitive impairment, behavioral health needs, medical needs, mobility limitations, hoarding concerns, housing instability, or limited natural supports.
  • Ability to work in a culturally sensitive, non-judgmental, and client-centered manner with a multicultural population who may have complex needs; work independently and as part of a team while maintaining professional boundaries.
  • Must follow documentation, confidentiality, HIPAA, APS, County, funder, and JFSLA requirements.
