SUMMARY

The Los Angeles County Department of Homeless Services and Housing (HSH) consolidates our countywide response to homelessness.

The driving force behind HSH is increasing accountability and transparency, improving care for people experiencing or at risk of homelessness, and streamlining collaboration with partners including services providers, the County’s 88 cities, and unincorporated areas to deliver high-quality, life-saving care. Staff schedules are based on business need and may include the option of a hybrid work schedule where employees work remotely and from the office.

The Homeless Prevention Unit (HPU) is a collaborative effort between HSH, California Policy Lab (CPL) and the Department of Mental Health (DMH). The HPU proactively identifies individuals who are most at risk of homelessness through predictiveanalyticsand provides them with flexible, early interventions to resolve issues that might lead to loss of their housing.

HPU is expanding to serve young adults between the ages of 18-24 in need of proactive homelessness prevention resources. The HPU has significant interactions with several County departments toaddress challenges navigating service systems and/or toconnect or re-connect individuals to services to ensure housing stability;therefore, the ideal candidate will be knowledgeable in mainstream and specific programs and services tailored to young people at risk of or experiencing homelessness while being flexible to meet the changing needs and demands of the program and this population.

The Assistant Staff Analyst (Transition Age Youth (TAY) Case Management Program Manager) will serve as a supervisory program manager for the HPU TAY program and will report to the Associate Director. Working closely with HPU program management and HSH management, this position will be responsible for developing and implementing program structures, processes and evaluation measures and will assist in managing day-to-day operations for the Unit.

The TAY Case Management Program Manager will supervise a team of TAY Case Managers who interface directly with HPU TAY clients and will be responsible for designing and implementing policies and procedures to assist clients with complex mental health needs, legal barriers, income barriers and/or housing insecurity. This role will require building a new team who is tasked with serving young people in Los Angeles County through a client centered model and developing policies and guidance that will help shape the TAY program of the HPU.

The ideal candidate has experience working with TAY who are/have experienced homelessness or at risk of homelessness. And has a deep understanding of evidence based best practices such as Trauma Informed Care and Harm Reduction.

ESSENTIAL FUNCTIONS
  • Oversees a team of HPU Case Managers and supports their efforts through regular supervision check-ins to determine and implement interventions and linkages to housing and services for HPU TAY clients.
  • Supports HPU Case Managers and their clients to identify social, emotional, and economic factors which may interfere with obtaining maximum benefits from medical and mental health care.
  • Reviews and monitors client outcomes with Case Mangers to address gaps in program delivery, documentation, and data input.
  • Supports individual Case Managers in identifying their strengths, areas of growth and helps facilitate professional development.
