This position reports directly to the U.S.VETS West Los Angeles Campus VASH Team Supervisor and Project Director. The HUD/VASH Case Manager provides guidance to veterans in VASH to achieve their highest level of independence. Case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates services to optimally meet each client’s needs.

Responsibilities
  • Provide case management and care coordination to facilitate appropriate delivery of health care services, incorporating complex multiple causation in the differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice
  • Behave professionally, tactfully, and mindfully of the local environment when interfacing with clients and partners
  • Assist with urgent client issues outside of business hours as needed
  • Possess fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures
  • Work with veterans and their families experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial challenges
  • Facilitate client intake process and compile client files ensuring all program eligibility documentation is complete
  • Maintain confidentiality in compliance with HIPAA standards
  • Assist the Program Coordinator and Clinical Director with auditing client documentation such as eligibility, case notes, IAPs, and discharge forms
  • Assist in program reporting
  • Supervise student interns, assigning tasks, reviewing recordings, conducting supervision sessions and evaluations, and adhering to university standards
  • Assist with training new staff and providing training to team members
  • Assess veterans referred to HUD/VASH holistically, identifying problems, needs, or barriers in housing, employment, education/training, financial, sobriety, legal, family/social, and medical/psychiatric domains
  • Examine psychosocial factors to determine level of functioning and degree of impairment across multiple domains, implementing clinically‑indicated treatment modalities
  • Evaluate and assess lethality, perform Mental Status Exams, and identify psychiatric manifestations of medical conditions
  • Collaborate with clients to complete Individual Housing and Employment Plans and Mental Health Treatment Plans
  • Have knowledge of criteria and state laws for legal commitment and involuntary hospitalizations
  • Assess and intervene with patients suspected of abuse
  • Provide counseling or psychotherapy to individuals, families, and groups
  • Provide specialized case management and life‑skills groups/classes to veteran subpopulations as applicable (e.g., women veterans, disabled veterans, Iraq/Afghanistan veterans, caregivers, families)
  • Facilitate housing stability supportive services, including Resident Town Halls, recovery groups, recreational activities, life‑skills groups, substance‑abuse groups, community resource referrals, peer support, faith‑based services, and employment services/opportunities
  • Conduct in‑home, community, and in‑office visits to provide ongoing reassessments
  • Establish and maintain effective working relationships and communicate with clients, staff, and community agencies
  • Meet program requirements for client meetings and document progress notes in DAP format
