RN - Case Management

This is a 13-week contract position.

Monday through Friday with one weekend per month. The weekend shift consists of 4 hours on Saturday and 4 hours on Sunday (with a flex off on the following Thursday).

The Clinical Liaison will play a vital role in coordinating patient care and facilitating transitions to and from inpatient rehabilitation. This role involves understanding post-acute care, care coordination, and building relationships with key stakeholders to enhance patient outcomes.

• Coordinate care for patients transitioning to and from inpatient rehabilitation.
• Engage with stakeholders to facilitate effective communication and ensure quality patient care.
• Assist in business development and marketing initiatives related to rehabilitation services.
• Float to cover different facilities as needed, which may involve local travel.

• 1-3 years of relevant experience in post-acute care, care coordination, or case management.
• A clinical background in therapy, nursing, social work, or related fields is required.
• Knowledge of insurance authorizations and related processes is preferred.
