Case Manager- LVN

Salary: $90,000 - $100,000 / year (Depending on experience)

Vasona Creek Healthcare Center is seeking a dedicated and organized Licensed Vocational Nurse (LVN) to join our team as a Case Manager. In this role, you will coordinate patient care, manage utilization reviews, and oversee discharge planning to ensure high-quality outcomes. You will act as the primary liaison between patients, families, healthcare providers, and insurance companies.

• Coordinate patient admissions
  , transfers, and discharge planning.
• Conduct utilization reviews to ensure appropriate level of care and insurance reimbursement.
• Collaborate with physicians
  , therapists, and nursing staff to develop individualized care plans.
• Communicate with insurance providers to obtain authorizations and manage concurrent reviews.
• Serve as an advocate for patients and families, guiding them through the healthcare continuum.
• Maintain accurate documentation in accordance with state, federal, and facility regulations.

• Active LVN license in the State of California.
• Minimum 1-2 years of case management or utilization review experience (preferred).
• Experience in skilled nursing (SNF) or long-term care settings highly desired.
• Strong knowledge of Medicare, Medi-Cal, and managed care insurance processes.
• Excellent communication

• Proficiency with electronic health record (EHR) systems.
