General Summary
Under the supervision of the Director of HIV Prevention and Treatment, the HIV Prevention Coordinator works collaboratively among a team of professionals in the Infectious Disease () department to support St. Thomas Community Health Center’s mission to reduce the spread of HIV in the Greater New Orleans metropolitan area. This coordinator will be responsible for:
Patient access to HIV pre‑exposure prophylaxis (PrEP) and a clinical resource for patients seeking or taking PrEP and for St. Thomas’s prescribing healthcare providers;Treatment as Prevention (TasP) in collaboration with the HIV treatment team to provide counseling and HIV testing for persons without HIV‑1 infection and counseling on transmission reduction for persons with HIV‑1 infection who are not virally suppressed; andCommunity outreach to establish partnerships within the Greater New Orleans area to market St. Thomas Community Health Center as a referral source for HIV prevention and treatment services.Responsibilities
Serve as an initial point of contact for patients and providers for HIV PrEP.Maintain a database for all patients on PrEP mas Community Health Center.Monitor the care and follow‑up of patients on PrEP as requested by the program director and referring healthcare providers.Present HIV prevention data and analysis as requested by the program director.Provide data entry and analytical support for select grant reporting requirements.Work as part of a team to develop, implement, and maintain processes for the improvement of St. Thomas’s HIV prevention services.Coordinate with other community‑based organizations to maximize prevention services and to conduct HIV‑related outreach in areas of high need with a focus on the priority population of women of color.Participate and confer with other members and healthcare providers as they relate to HIV prevention.Go into the field when necessary, to promote the HIV prevention services offered by St. Thomas Community Health Center.Accurately track and document communication with patients and patient‑related entities.Continuously expand knowledge and understanding of community resources, services and programs, provided; schedule outreach/education events in the community and conduct health education workshops at the clinic and in the community individually or as part of the .Follow all HIPAA compliance and regulations in accordance with local, state, and national laws.May perform other duties as appropriate for the care of persons at risk for or living with HIV‑1 infection within the scope and capacity of the level of education and skill set obtained and/or as directed by the Director of HIV Prevention and Treatment and other St. Thomas’s administrators.Qualifications
Bachelor’s degree from an accredited four‑year college or university.Experience of interest in public health or HIV‑related professional work.Strong interpersonal skills, excellent public speaking abilities, strong written and verbal skills.Excellent organizational skills.Skilled with Microsoft Excel®, PowerPoint®, Word® or other appropriate computer software.Ability to analyze, report, and use data. Ability to enter data and perform clerical duties including data audits, file management, and general office skills.Ability to multitask and work collaboratively as part of a team.Valid Louisiana driver’s license, reliable transportation, and the ability to drive locally to different locations for field responsibilities.Ability to perform all tasks and job responsibilities safely without injury to self or others in compliance with St. Thomas Community Health Center policies and procedures.Willing to commit to the mission, vision, philosophy, and goals of the St. Thomas Community Health Center.#J-18808-Ljbffr
