Job Summary- Supervise social service programs, assess residents' social needs, and develop and update personalized care plans.
- Refer residents and families to community resources, assist with adaptive equipment, and coordinate services.
- Support residents with financial, legal, and discharge planning, including coordinating outside services and ensuring proper documentation.
- Assist with admission and discharge processes, including placements and transfer arrangements.
- Full-time position with negotiable salary ($45,000 – $64,000 annually).
- Benefits include 401(k), health, dental, vision, life insurance, and paid time off.
- Work is conducted in person, adhering to federal and state regulations and company policies.
