The Medication Coordinator assists in the implementation of the delivery of health care services by responding to medication refill requests of patients. He/She processes refill requests electronically and ensuring all information is accurate and complete. He/She demonstrates actions consistent with St. Thomas Community Health Center’s Service Principles as duties are performed on a daily basis.
Minimum Qualifications- Education, Experience & Training:
High School Diploma or GED 3+ years of human services experience, preferably in pharmacy services Must be over 21 years old. BLS, preferred;
Knowledge of over-the-counter medicines and common prescriptions; required - - Other
Qualifications:
Ability to work efficiently under pressure, ability to set priorities, self directed and ability to maintain strict confidentiality.
- Ability to develop a rapport with patients.
- Medical terminology.
- Knowledge of computers.
- Telephone skills.
- Strong interpersonal skills.
- Good organizational skills.
Job Duties
- Completes medication refill request or electronic prescribing of medications based on the clinic’s protocol for prescription refills.
- Demonstrates knowledge of commonly prescribed medications.
- Completes tasks related to medication prescribing and refill requests.
- Demonstrates knowledge of financial assistance available to patients and informs patient(s).
- Submit prior authorizations, as necessary.
- Collaborates with pharmacies to ensure patient medications are prescribed and/or delivered, as appropriate.
- Demonstrates knowledge of insurance plans, specifically regarding those medications that are covered or are not covered by the plan.
- Ensure all tasks provided and associated with patient care, patient administrative processes, and related duties complies with all regulatory and accreditation standards, as well as clinic policies and procedures.
- Documents any patient interaction accurately in the EHR (messages, phone calls, etc.)
- Returns patient phone calls within 24 to 72 hours.
- Checks and responds to tasks, phone messages in a timely manner.
- Utilizes computer system to retrieve medical information.
- Completes all paperwork/computerized information related to patient care and medications.
