As a Senior Care Coach supporting our Illinois Health Choice Medicaid program, you'll play a dual-impact role: helping Medicaid members achieve better health outcomes while mentoring and supporting a team of Care Coordinators who make that care possible.
You will serve as a trusted resource, providing coaching, training, and guidance to team members, supporting onboarding and development efforts, and helping ensure member-centered care. You'll collaborate with interdisciplinary teams, monitor member progress, connect individuals and families with valuable community resources, and help drive positive outcomes through proactive care planning and advocacy.
Upon hire, you will be assigned to support one of the following specialty areas:
Maternity, Pediatrics, Physical Health, or Behavioral Health.
This is an exciting opportunity for someone who is passionate about leadership, collaboration, and improving the health and well-being of Illinois Medicaid members.
- Provide coverage and support for Care Coordinators and be able to document in the clinical system.
- Mentor and support the team, as well as training initiatives.
- Support new hire onboarding including but not limited to precepting and case audits.
- Provide daily reports to the Care Coordinators regarding outstanding cases and review daily compliance dashboards.
- Support audit initiatives and support any internal and external auditing processes for the team.
- Complete field work support and review reports, as needed.
- Coordinate, monitor, and evaluate the case management plan against the member's personal goals.
