Physical Therapist - PRN

Role Overview: The Physical Therapist in Home Health assesses and evaluates patients' functional status, mobility, endurance, pain, and medical conditions. They develop and implement treatment plans to improve patient well-being, monitor progress, and adjust care as needed.

• Evaluate patients' physical and systemic health, collaborating with physicians on care plans.
• Provide therapy within legal and professional scope, coordinating with healthcare team members.
• Observe and report patient responses and condition changes.
• Administer treatments to relieve pain, restore, or maintain function using evidence-based methods.
• Educate patients, families, and healthcare team members about care plans, limitations, and progress.
• Conduct patient visits across service areas, documenting progress and outcomes.
• Complete evaluations and develop individualized plans of care per state guidelines.
• Participate in staff meetings and committees as needed.
• Maintain clinical knowledge and licensure through ongoing education.

• Licensed in Physical Therapy in the relevant state.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
