Nutrition Care Coordinator
Interview and screen patients, modify a variety of both standard and nonstandard diets, coordinate nutrition care activities with nursing services, and instruct patients regarding menus.
Perform a variety of administrative and clerical duties.
Assist the dietitian in nutrition screening and assessment of individual patient needs by evaluating diet history, obtaining medical information and patient tolerances and allergies from the medical record and via interview.
Record patient information in the medical record with the up-most accuracy and attention to detail per Nutrition Care policy.
Obtain diet orders from ward nurses or systems and ensure accuracy of the diet orders by communicating with nursing staff regarding diet changes and alternate tray delivery times pending the patient's schedule.
Evaluate and uses information to assign each patient a nutritional risk category.
Conduct admission nutritional risk screening procedure on all new patients.
Provides dietary information both through oral and written instruction methods to patients, their significant others, and nursing staff.
