Signify Health is seeking a Full Time Clinician (NP or PA) to provide In-Home Health Evaluations across assigned coverage areas. Travel up to 100% of the time with potential overnight stays. You will conduct medication and medical history reviews, a physical exam, and diagnostic tests per health plan orders using an iPad workflow.
